Just $15.00 for individual plan or $25.00 for a family plan for two adults and two children. A program that will allow you to have access to high-quality laboratory testing from blood work, diabetes, cancer, drug testing, anti-aging, hormonal, std’s, and many other available tests. There is no need for insurance or employer involvement in the process. If you are uninsured or have high insurance deductibles CareLabs4Less can help you obtain the test you need when you need it. Through our CareLabs4Less private portal you will have a completely secured and discreet account that will allow you to be in control of your healthcare and wellbeing.

Q: If I feel healthy, why should I get tested?

Q: How do I process a laboratory testing request?

A: 1. Order Lab Tests Online (A Desktop, Tablet, or Smartphone) Our website and mobile online portals provide a wide-ranging list of testing choices including comprehensive packages at discounted member pricing. Once you have selected your online lab tests and nearest lab location, you will be re-directed to complete your order by allowing you to select your payment using one of our various payment options. Our portal is private and completely secured for your privacy and protection. Upon payment approval you will be provided with your Laboratory Medical Confirmation number and information on your lab collection site. If for any reason have any further questions you may contact our customer service line during working hours and speak to one of our representatives by calling: 1-XXX-XXX-XXXX. 2. Go To Your Selected Laboratory Location Once our clients receive their medical order confirmation information, they are able to go into the selected laboratory at their convenience during their posted business hours of operation. No appointment is necessary as our labs operate on a walk-in basis. Clients may go to the lab for collection during the lab’s regular business hours arriving at least a thirty-minutes before any lunch or closing scheduled times. 3. Get Your Results Privately Members are given several options to obtain their test results. Our members can obtain the test results by logging in to their private portal account securely. By request, we are also able to provide our members/clients with a paper copy of their test results by mail or by also forwarding it to their respected physician or healthcare professional.

Q: Can I cancel or get a refund on my order?

A: To cancel your order, you must submit your request by calling CareLabs4Less at 1-XXX_XXX_XXXX or submitting your request via email at customerservice@ CareLabs4Less.com. Please allow 24-72 hours for us to process your request. Refund Policy: For orders not redeemed at the laboratory. 1. From the payment date until 90 days, a full refund to the original form of payment. 2. Orders which are more than 90 days old are considered void, may not be redeemed for services and are no longer applicable for refunds or credits of any kind.

Q: Do I need to schedule an appointment?

A: For the most parts no appointment is needed at the laboratory collection site which you have selected. However, we encourage all clients to check the locations business hours as well as, patient service guidelines for that location. Once your testing order is completed and paid for you will receive your order confirmation number, you may then walk into the selected lab during their business hours. We ask out of courtesy to the lab attendants, that you arrive at least 30 minutes before their lunch break (if any) or the close of the business day.

Q: Are there any special requirements before I give my specimen at the lab?

A: Some laboratory test do have special requirements. There will be a notation if there is a specific specimen requirement for your selected laboratory test.

Q: Should I continue to take my medication before being tested?

Q: I feel nervous and embarrassed to go to the lab for testing. What should I expect?

A: When you arrive at the lab collection site, you will provide the attendant with your medical order confirmation number. If there are other customers waiting, be assured that no one knows which test you are taking except the laboratory staff. The laboratory patient service locations provide a waiting room and several small specimen collection rooms. The lab collects specimens for hundreds of types of tests at each of their collection centers.

Q: How do I receive my test results?

A: Results can be viewed online by logging in to your secure private account at your convenience. Please note that we are Not able to email test results directly due to our privacy guidelines.

Q: Will I be able to understand my test results?

A: Yes, all of our test results include reference interval ranges and any abnormal results are clearly specified. We highly recommend you discuss any test results with a physician or healthcare provider.

Q: What if my lab results are abnormal?

A: If for some reason you find that your test results include an Abnormal reading, we highly recommend for you to consult with your physician or healthcare professional. Abnormal test results should be considered an early warning, not necessarily an indication of illness or disease – but, once again we encourage a proper follow-up is done with your healthcare provider.

Q: Does Request A Test provide testing for minors?

A: CareLabs4Less Minor testing policy is as follows: 1.) STD testing 16 years and older 2.) Drug (toxicology) testing, minor testing is permitted subject to CareLabs4Less approval based on test selection and circumstance. Please feel free to email Customerservice@CareLabs4Less.com with questions. Online orders will be reviewed, and we may connect with the order party to further discuss. 3.) All clinical lab tests, other than STD testing (see item 1 above), our clients must be 18 years of age or older

Q: Can I get the results of each test I order on a separate report?

A: All tests which are on the same order will have results on the same report. If you require your test results to be on separate reports, please order and pay for them separately. The lab will be unable to separate results for tests which are ordered together.

Q: Does CareLabs4Less offer lab testing on animals?

A: Unfortunately, our service is unable to provide testing for animals. The labs we partner with will not accept any non-human specimens.
